Interpreter Role & Responsibility / Document Requirements

Submit the following documents to Universal:

UniversalLanguage document requirements:
- - Valid photo ID (e.g. driver’s license)
  - W-9 Form
  - Independent Contractor Agreement
  - Business Associate Agreement Form
- HCA Contract document requirements:
  - Signed copy of the Interpreter Role & Responsibility
  - Signed copy of the Interpreter Orientation Attestation
  - Signed copy of the Language Interpreter and Translator Code of Professional Conduct
  - Annual National Background Check
  - Language Access Provider has the option to provide a valid current background check annually
  - Washington State Unified Business Identifier (UBI)
  - Immunization records as outlined in the HCA Interpreter Orientation:
    - MMR
    - Varicella
    - Tdap
  - Annual Flu Shot
  - Annual TB Screening
  - COVID Vaccine (may vary by facility)
  - BAA and HIPAA Documents
- Credentials:
  - DSHS Certification
  - DSHS Authorization, or
  - DSHS Recognized Certification
Complete job invoice electronically or in paper form (as indicated by the provider)
Comply with HIPAA, the DSHS Code of Ethics, any other applicable state and federal laws, as well as all terms and conditions of Contract K2474
Maintain an active and valid Washington State Unified Business Identifier (UBI) number or tax registration number
Any additional requirements as deemed necessary by HCA and/or the Collective Bargaining Agreement; interpreters will be notified in a timely manner of any such requirements
